Request South Guthrie Community Center for Your Event
Completion of this form implies legal responsibility of compliance with all conditions of use as outlined by the Parks & Recreation Department. We strongly recommend you bring a copy of your e-mail receipt with you to your event for proof of reservation.

Deposit: $100 due within twenty-four (24) hours of receiving Parks & Recreation request approval.
Rental Fee per day: $150 (due 30 days prior to event date)
Refund: $100 deposit mailed within ten (10) business days after event date.
Tables/Chairs: Renter is responsible for setup of tables and chairs.
